• Where do you offer personal training and massage?
    We come to you! Home, office, school, special events or health fairs are the most popular places for massage or personal training. Scheduled appointments in and around Chicagoland make the highway our home, so when you reach the voicemail system, we're with a client or part of an event. Your call will be returned 9 am to 9 pm Monday through Saturday. Emails generally receive replies early in the morning.

  • How can I prepare for my personal training session?
    If you are a new client, you will need to complete and submit signed copies of an intake form, an exercise readiness questionnaire and, in some cases, a medical release form. A doctor's signature may be required prior to your fitness assessment test.

    If you are training for an event, let the trainer know more about your event(s) and where you are in your training cycle for your sport. A training cycle (periodization cycle) can be created for you if needed.

    Any special concerns you have at any particular session should be mentioned to the trainer so special focus can be applied to your needs.

    If you have a sports-related injury, or for that matter, anything that has changed since your initial form(s) were completed, please advise the trainer of such conditions.

    Be sure to eat a balance meal about 2 hours before your workout. A small snack 45 minutes prior to a workout is advised. Have water on hand before, during, and after your workout.

    Breathable gym attire and well-fitting shoes are required.
  • How can I prepare for my massage?
    If you are a new client, you will need to complete an intake form. Be sure to allow about 15 minutes to read, complete, and sign the form the massage therapist will provide.

    Massage for relaxation will not require anything more than easing your mind and body into a state of reduced stress. Clear you mind, let your body be lose.

    If you are receiving a sports massage, let the therapist know more about your event(s) and where you are in your training cycle for your sport.

  • What qualifications do your personal trainers have?
    All of our trainers hold accreditation as Certified Personal Trainer (CPT) and Certified Nutritional Consultant as well as maintaining CPR Certification.
  • What qualifications do your massage therapists have?
    Each massage therapist from Amplify Fitness is Illinois state licensed and nationally certified by NCBTMB. In addition, each massage therapist must maintain accreditation with one of the two nationally recognized professional bodywork associations, ABMP or AMTA.
  • Do you accept credit or debit cards for payment?
    We currently accept credit and direct bank payments through Paypal as prepayment for massage packages and personal training packages.

    For individual services, cash or personal checks are the only payment methods accepted.
  • Will my healthcare plan pay for personal training or massage?
    Check with your provider, as more and more insurance companies are paying for at least part of costs associated with regular theraputic massage. Some healthcare providers will cover expenses related to personal training services as part of a strategy for clients with unique health concerns.

    Amplify Fitness currently does not accept insurance or other healthcare payments. We are glad to write out a full description of each session so that you may attempt to be reimbursed by your insurance company.
  • What is your cancellation policy?
    For all services, a 24-hour notice by phone is required.
