Prior to a massage session, we need to know some basic information about you. The form below can be downloaded, printed, and completed by you and submitted to us at your appointment. We ask that you complete this form so that we may provide a customized massage created to meet your needs. Should your doctor recommend massage for a particular reason, please download, print, and have your physician complete the "physician_permission" form as well.
